After losing her 53-year-old brother to heart disease and witnessing patients face overwhelming barriers to managing chronic conditions, we founded Will’s Heart to make a difference.
We provide patient navigation, education, advocacy, and trust-building to help individuals overcome bias, system gaps, and access challenges—so no one has to face their health journey alone.

We offer patient navigation to assist with medical appointments, medication access, and understanding health benefits. Our certified case manager works closely with each client to support blood pressure management, medication understanding, and overall health goals.
Through care and practical guidance, we help our clients build healthier lives and successfully return to their communities.

Assistance in scheduling primary care appointments, specialty care visits, and follow-ups. Accessing medications and gain insights into health benefits for comprehensive care support.
Independent RN Case Management Program
Registered Nurse- led program focusing on managing blood pressure, medication understanding, and health goal achievement through personalized care and step-by-step guidance towards improved health outcomes.
